Dr Don Rix Award for Physician Leadership Nomination Form

Thank you for your interest in submitting a nomination for this award. As a reminder, all nominations must include:

• One nomination letter clearly describing why the nominee is deserving of this award
• One letter of support detailing the nominee’s contributions and/or achievements
• The nominee’s current curriculum vitae

Please note the nomination letter and letter of support should clearly describe why the nominee is deserving of this award – how they have achieved distinction in one or more of the following areas:

1. Supported lifelong learning opportunities.
2. Promoted excellence in medical education.
3. Built consensus among physicians and groups of physicians.
4. Provided leadership for new initiatives both in business and clinical practice.
5. Provided leadership and service to the general community or province either by direct support or through philanthropy.
6. Provided service to the medical profession through participation in Doctors of BC.
7. Provided leadership to the broader medical community.
8. Participated in legislative and other political activities in support of health care.

The deadline for nominations is May 31, 2024.
