CMA Honorary Membership Award Nomination Form

Thank you for your interest in submitting a nomination for this award. As a reminder, all nominations must include:

• A nomination letter clearly detailing the nominee’s contributions and/or achievements
• The nominee’s current curriculum vitae

Please note the nomination letter should clearly describe how the nominee meets the nomination criteria:

1. Held in high regard by their colleagues
2. A humanitarian who has put into practice the aims and ideals of the profession
3. Someone who exemplifies the words of the CMA Coat of Arms: "Integritate et misericordia" - integrity and compassion
4. A member in good standing of the Canadian Medical Association (CMA) for at least 10 years
