Application for Insurance Wizard

At Doctors of BC Insurance, we have multiple forms to apply for new insurance coverage, depending on your career stage and what you are applying for. This wizard will ask you 1-3 questions to guide you to the correct form you should complete in your situation.

Note: if you are doing a non-medical switch of your insurance at a transition time (i.e. graduating medical school and moving into residency or completing residency/fellowship and moving into practice), this wizard does not capture the "options forms" that we use to capture insurance changes that do not require a medical.
