Health Spending Account

Please use the Health Spending Account Open Enrolment Application: It'll take about 15 minutes to apply.

You'll need the following:

• names, dates of birth, and provincial health coverage for all participating physicians and medical office staff employees on the Account.
• your Account name and Business ID (found on your most recent Health and Dental invoice)
• if setting up HSA for employees, you'll need the descriptive names for the Employee Classes you want to set up (only needed if having multiple Employee Classes).

 

When enroling during the open enrolment period, the effective date for the new HSA will be January 1 of the following year.

Thanks for confirming that you are recently incorporated. If your date of incorporation is within the past 6 months, you can add the HSA effective the 1st of the month following receipt of the signed applicaiton.

Please complete a "change" request using the member agreement.

Indicate that you are applying for a 'change to existing application' and that you are recently incorporated and are adding HSA and switching to a corporation.

To terminate only your health spending account or to request a reduction in your HSA account limits, please complete a request for termination and complete the relevant HSA portion.

(this form is coming January 2027)

Increases to HSA account limits are only allowed during the Annual Options Period that runs September 1-October 31 each year.

To increase the HSA account limits for either Physicians or Employee classes, please complete the Annual Options Period changes form to make adjustments. (this form is coming in 2027)

To submit a rename of existing Employee Classes, please complete this Schedule A amendment form. (this form is coming in 2027)
